- The information provided on www.definept.com, including text, graphics, images, and blog content, is for general educational and informational purposes only.
- Not Medical Advice: Browsing this website or submitting a contact form does not establish a physical therapist-patient relationship.

2. Direct Physical Therapy Access (California Law)
- In accordance with California law (California Business & Professions Code § 2620.1), patients in California may initiate direct physical therapy treatment directly from a licensed physical therapist without a physician's referral.
- Direct access treatment is limited to 45 calendar days or 12 visits, whichever occurs first.
- Beyond 45 calendar days or 12 visits, continued physical therapy care requires an in-person or telehealth examination and a signed plan of care approval from a licensed physician, podiatrist, or surgeon.
3. Online Appointments & Third-Party Platforms
- Jane App Scheduling: Patient portals, clinical scheduling, intake forms, and account registrations are processed securely through Jane App. Your use of the online booking portal is subject to Jane App’s security standards and terms of service.
- Marketing & Event Registration: Forms used for wellness workshops, special events, or email sign-ups may be hosted via HubSpot.
4. Intellectual Property & Acceptable Use
- All content on definept.com (logos, images, written material, design) is the property of Define Wellness and Physical Therapy, Inc. (and its affiliated DBAs) and is protected by intellectual property laws.
- You may view, download, and print pages for your own personal, non-commercial use.
- You may not copy, reproduce, or redistribute website content for commercial purposes without prior written consent from Define Wellness and Physical Therapy, Inc.
5. Mandatory Binding Arbitration & Class Action Waiver
- PLEASE READ THIS SECTION CAREFULLY. IT AFFECTS YOUR LEGAL RIGHTS, INCLUDING YOUR RIGHT TO FILE A LAWSUIT IN COURT.
- Binding Arbitration: You and Define Wellness and Physical Therapy, Inc. (including its DBAs Define Physical Therapy and Define Wellness) agree that any dispute, claim, or controversy arising out of or relating to your use of this website, online services, or digital data collection shall be resolved exclusively through final and binding individual arbitration rather than in court.
- Governing Rules: The arbitration shall be administered in Orange County, California, under the Consumer Arbitration Rules of the American Arbitration Association (AAA).
- Class Action Waiver: YOU AND DEFINE WELLNESS AND PHYSICAL THERAPY, INC. AGREE THAT EACH MAY BRING CLAIMS AGAINST THE OTHER ONLY IN YOUR OR ITS INDIVIDUAL CAPACITY, AND NOT AS A PLAINTIFF OR CLASS MEMBER IN ANY PURPORTED CLASS, CONSOLIDATED, OR REPRESENTATIVE PROCEEDING.
6. Limitation of Liability
- Define Wellness and Physical Therapy, Inc. (and its DBAs), as well as its licensed physical therapists, officers, and staff, shall not be liable for any direct, indirect, incidental, or consequential damages resulting from your use or inability to use this website or any external links provided.
7. Governing Law & Jurisdiction
- These Terms and Conditions are governed by and construed in accordance with the laws of the State of California, without regard to its conflict of law provisions. Any legal action not subject to arbitration shall be filed in the state or federal courts located in Orange County, California.
